Service Objectives 

  • To provide a package of integrated home-based support services for people with severe disabilities through the professional assessment of a multi-disciplinary team and the adoption of a case management model to meet their care, training and nursing care needs; training and support are also provided to the family members or carers to raise their caring capability and relieve their caring burden so that they can have a better quality of life.

Target Group

  • People with severe mental or physical disabilities living in Tuen Mun District or Kwai Tsing District who are on the waiting list for subvented residential care service of Hostel for Severely Physically Handicapped, Hostel for Severely Mentally Handicapped or Care and Attention Home for Severely Disabled Persons; students attending special schools for children with severe intellectual/ physical disabilities; and family members/ carers of the above-mentioned service users.

Scopes of Service

  • Personal Care: transfer, personal hygiene, feeding or assistance to feed, meal support, dressing and changing clothes, bath or shower, grooming, incontinence care and so on.

  • Nursing Care: observation of vital signs (blood pressure, pulse, body temperature and body weight, etc.), guidance on medication, gastric tube feeding, care of urinary catheter, wound, stoma, incontinence, respiratory tract, diabetes, etc., control of infection, peritoneal dialysis and so on.

  • Rehabilitation Training: rehabilitation exercise, general exercise, exercise recommended by occupational therapists or physiotherapists, home environment assessment and home modification assessment, etc.

​

  • Case Management: Tailor-made individual service plan and provide counselling to handle emotional and behavioural issues; if necessary, referrals can be made to refer service users to other social service organisations or medical services.

​

  • Escort: service users are accompanied to and from hospitals, clinics, schools and rehabilitation units for services or participating in the activities.

​

  • Support for Carers: provide training to strengthen the caring capability; organise programmes and mutual-support groups to develop support network with people in similar situations.

​

  • Home Respite Service: provide temporary home-based support service.

 Fees  

  • Full package of services: HK$1002 per month

​

  • Personal care service: HK$33 per hour

​

  • Occupational Therapist or Physiotherapist service: HK$52 per 45 minutes

​

  • Nursing care service: HK$43 per 45 minutes

​

  • Fees may be charged depending on individual activities.
