Service Objectives

Elders may choose community care services that suit their individual needs with the use of Community Care Service Voucher for the Elderly. The centre assists them in maintaining their motive and body functions and improving their quality of life. Support is given to carers of elders to continue their role so that eventually “age in place” can be achieved.
Target Group
-
Eligible participants are elders who have been assessed by SWD’s Standardised Care Need Assessment Mechanism for Elderly Services to have impairment at moderate or severe level and are waitlisting for subsidised community care services (CCS) and/or residential care services (RCS) on the Central Waiting List for Subsidised Long Term Care Services without any kind of RCS or subsidised CCS being received; people aged 60 or above living in Sham Shui Po, Kowloon City or Yau Tsim Mong with “Community Care Service Voucher for the Elderly” issued by the Social Welfare Department.
Scopes of Service
In the mixed mode, the centre provides centre based and home based services as follows:

Care Plan: Assessment plan is set out, implemented and reviewed by nurses, physiotherapists and occupational therapists.

Nursing Service: our nurses provide nursing care (e.g. medication and monitoring etc.), health evaluation, advice on rehabilitation and health education.

Rehabilitation Therapy: physiotherapists and occupational therapists tailor-make rehabilitation exercises and therapeutic plan for elders to promote exercising activities and provide advice on using and lending rehabilitation equipment so as to improve elders’ mobility and body function.

Psychological Social Health Service: social workers provide counselling and referral service to assist elders in adapting to communal life and ease issues in relation to emotional, financial and personal relationship; the centre also organise various interest, developmental, educational and volunteer groups for elders to better spend their leisure time.

Home Care Support: to provide meals and assist elders in their living such as toileting and feeding.

Living Care: to deliver meals, escort, personal care (e.g. bathing and simple care), rehabilitation exercise and household cleaning and so on.

Support to Carers: to organise various workshop to share skills of taking care of elders such as caring and rehabilitation tips after stroke, manual-handling technique, home care of patients with diabetes or dementia and so on; in addition, to build a support network among the carers and organise interactive groups.

Activity room, nursing room, sensory training room, therapy and exercise room, pantry, meeting room, TV, touch-screen computer, audio and visual equipment, newspapers and magazine
Fees
-
The elderly will pay for the co-payment amount as assessed by the Social Welfare Department.
Application Procedures

Visit in person or call to the centre for application; or refereed by government departments, the Social Welfare Department, hospitals, welfare organisations, local groups or individuals.
