Service Objectives
-
Through the provision of professional services with love, care and respect, the Home holding the continuum care and whole-person belief aims at providing a home-like and comfortable environment to elders who are physically weak and not suitable for living at their home. The elderly can enjoy all-round residential care services and have a healthy and rich golden age.
Target Group
-
Government-subsidised places: Elders aged 65 or above with a moderate level of impairment as assessed under the Standardised Care Need Assessment Mechanism for Elderly Services of the Social Welfare Department. The elderly are usually physically weak with impairment and cannot take care of their daily living by themselves. They should be suitable for a communal living.

Self-financed places: women aged 60 or above.
Scopes of Service

Care Plan: a physiotherapist, an occupational therapist and social workers assess, set out, implement and review the care plan for the residents.

Medical Care: medical consultation service and health check provided by a registered medical practitioner; nursing services, including administration and supervision of medication, health assessment, rehabilitation advice and health education.

Rehabilitation Therapy: a physiotherapist and an occupational therapist design therapeutic exercise and treatment to maintain or improve the physical and cognitive functioning of residents and to promote health exercise. Information on the use of rehabilitation equipment is provided to maintain the body function of the residents

Counselling and Activities: social workers provide counselling and referral service to facilitate residents in adapting into the communal living at the Home and solve their problems in relation to emotion, finance and social relationship; various interest groups of development, education and volunteers are held for elders to better use of their leisure time.

Daily Living Care: accommodation, meals and laundry services are provided within shared rooms; staff on duty 24 hours per day taking care of daily living such as bathing, toileting and feeding of residents.

Participation Emphasised: family meetings are held to strengthen a sense of belonging to the Home; keep in touch with family members to provide support to carers.

TV corner, reminiscence corner, nursing room, special nursing room, multi-sensory training room, therapy and exercise room, meeting room, bedroom equipped with washroom, kitchen, laundry, air-conditioners, furniture and electric appliances
Fees

Government-subsidised places: based on the standard set out by the Social Welfare Department.

Self-financed places: depending on individual health conditions and service needs.
Application Procedures

Government-subsidised places: elders or their family members may apply or request for referral through Integrated Family Services, Medical Social Services Units, District Elderly Community Centre, Neighbourhood Elderly Centres, Social Centres for the Elderly, or other units (e.g. Family and Child Protective Services Units).

Self-financed places: apply directly to our Home.
